Pilot error. Lt. Col. Arthur "Bud" Holland had been reprimanded several times for aggressive flying and not flying within USAF guidelines.
He had quite a few flight crews ask not to be assigned as his crew. He should of been grounded on several occassions, but for some reason his Commanders did nothing but verbal reprimands, if that!
Google him, you will find that he was not only aggressive in the plane but on the ground also, The word "Jerk" comes to mind when describing him.
